«&» should be used in icon paths

In order to be w3c compliant, we should use & instead of & in icon urls. We need to decide wether to write & in paths or encode them automagically at some other level.

Using htmlentities seems like the way to go. icon paths should be stored with & and converted only when displayed. The small problem with this is we need to find all uses of icon paths.
