Plugin header system should allow to have multiple plugins on the same page

Hello,

Plugin header system for (de)activation is quite restrictive, and always use the same name for the form button, therefore forbidding to have multiple plugin headers on the same page while it would be quite useful, for instance in connectivity

This add an optional parameter for button form name (it will be used by simplePlugin)
