Show the form URL in edit mode

Descriptive title for this enhancement

The idea will be to show "pubicform.php=form{cn}" when we edit a form so we directly know the form URL without checking the doc or the code

Actual behavior

Right now we need to check doc/code to find it

Expected behavior

See the url easily from the interface

Step by step description of new behaviour

1. Edit the form
2. The url is displayed somewhere in the interface
